Schedule
Call or text us to schedule. We’ll collect basic info and get you an appointment.
Paperwork
Arrive a few minutes early to complete intake paperwork. Bring your ID and insurance info.
Evaluation
Dr. Eldeeb will review your injury history, examine you, and discuss your symptoms in detail.
Plan
You’ll receive a clear treatment plan — what tests, procedures, or follow-ups are recommended and why.
Treatment
We proceed with treatment promptly — no unnecessary delays between evaluation and care.
What to bring to your first appointment
- Photo ID
- Insurance card (auto insurance / PIP card, or health insurance card)
- Any previous imaging (MRI, X-ray) on disc or digital copy
- A list of current medications
- Records from any previous treatment for this injury
- Date and description of your accident
Florida’s 14-day rule — important for auto accidents
Florida law requires that auto accident patients seek medical evaluation within 14 days of the accident to qualify for Personal Injury Protection (PIP) benefits under their auto insurance policy.
PIP covers up to 80% of reasonable medical expenses related to the accident. If you wait beyond 14 days, you may lose access to this coverage entirely.
