Patient questions

Do I need a referral to be seen at PainMed?

No. Patients can contact PainMed directly to schedule an appointment without first obtaining a referral from another provider. You can call us at 813-668-0000 or text us at 609-362-5073.

Do you provide transportation to appointments?

Yes. Transportation to and from your PainMed appointments is available. If you need a ride, let us know when you call to schedule and we will make the arrangements for you.

Do you treat auto accident injuries?

Yes — that is our primary focus. PainMed is designed specifically for personal injury patients, including people injured in auto accidents. We evaluate and treat neck pain, back pain, radiating pain, nerve symptoms, and other injury-related conditions.

Do you treat slip and fall injuries?

Yes. PainMed evaluates and treats patients with pain resulting from slip and fall injuries, in addition to auto accident patients.

How quickly can I be seen?

We prioritize getting injury patients evaluated promptly. In most cases, we can schedule new injury patients within the same week of their initial call. Call us at 813-668-0000 to check current availability.

Do you offer Saturday appointments?

Yes. Saturday appointments are available from 8 AM to 5 PM — ideal for patients who cannot take time off during the work week.

What should I bring to my first appointment?

Please bring your photo ID, insurance card (auto PIP or health insurance), attorney contact info if you have representation, any previous imaging (MRI, X-ray), a list of current medications, and prior treatment records if available. Also be ready to describe the date and circumstances of your accident.

Do you accept health insurance?

PainMed works with auto PIP coverage and health insurance for personal injury cases. Please call our office to verify your specific coverage before your first visit.

What is Florida’s 14-day rule for auto accidents?

Florida law requires that auto accident patients seek a medical evaluation within 14 days of the accident in order to maintain eligibility for Personal Injury Protection (PIP) benefits under their auto insurance policy. If you were in an accident and haven’t been seen yet — don’t wait. Call us immediately.

Do you offer Spanish-language services?

Yes. Spanish-speaking staff are available at PainMed, and Spanish-language intake forms are provided. Dr. Eldeeb also speaks Arabic.

Does the doctor speak Arabic?

Yes. Dr. Mohammad Eldeeb speaks Arabic fluently, which allows him to communicate directly with Arabic-speaking patients in Tampa’s diverse community.

Can I text your office?

Yes. You can text PainMed at 609-362-5073 for scheduling and general questions. Please do not use text messaging for medical emergencies or urgent clinical concerns — call 911 for emergencies.

How do I request medical records?

Records requests can be submitted through our online records portal. For urgent litigation-related requests, please call our office directly at 813-668-0000.

What credentials does Dr. Eldeeb hold?

Dr. Mohammad Eldeeb is board-certified in pain medicine (D.ABA, D.ABIPP) and holds the FIPP designation (Fellow of Interventional Pain Practice). He completed fellowship training in interventional pain management and previously served as Chief of Pain Medicine at Lakeland Regional Health. Full credentials are on the About page.

Do you provide impairment ratings and future medical cost projections?

Yes. These services are available for appropriate personal injury cases. Please contact our office to discuss eligibility and timing relative to your case.
